Bedaquiline falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Guggul interacts with.

Theoretically, guggul might reduce the effects of CYP3A4 substrates.
In vitro research shows that guggul constituents known as guggulsterones can induce CYP3A4.

Here's the concern with pairing guggul and bedaquiline. Bedaquiline is cleared from your body by a liver enzyme called CYP3A4. Lab studies hint that guggul (specifically its guggulsterone compounds) may speed that enzyme up. If that happens in real life, your body could break down bedaquiline faster, leaving less of it to fight the TB bacteria. That matters because bedaquiline is part of a carefully built combination to treat multi-drug resistant tuberculosis, and weakening it could risk treatment failure or resistance.

I'll be honest: this is based on test-tube research, not human studies, so it's theoretical. Still, because the stakes with MDR-TB are high, I'd play it safe. Please check with your TB doctor or pharmacist before using guggul.

Guggul is a gum resin from the Commiphora wightii tree, long used in Ayurvedic medicine for cholesterol, joint, and skin problems. What Is Bedaquiline Used For?

Brand name: Sirturo

Sirturo (bedaquiline) is used as part of a combination treatment for pulmonary TB — TB that affects the lungs — caused by bacteria resistant to at least both rifampin and isoniazid. It is approved for adults and for children 2 years and older who weigh at least 8 kg (about 17.6 lbs).

Sirturo is not used for latent TB (TB infection with no active symptoms), TB outside the lungs, drug-sensitive TB, or infections caused by non-tuberculosis mycobacteria. It must always be used alongside other TB medicines — never alone.
