Diltiazem falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Guggul interacts with.

Theoretically, guggul might reduce the effects of CYP3A4 substrates.

Here's the gist for Diltiazem: your liver uses an enzyme called CYP3A4 to clear this drug from your body. Lab studies hint that the active compounds in guggul (guggulsterones) may speed that enzyme up. If that happens in real life, your body could break down diltiazem faster, leaving less of it working. The likely result would be weaker blood pressure control or less relief from chest pain (angina), not more side effects.

Now, the honest part: this is based only on test-tube and animal research, so we don't really know how much it matters in people. I wouldn't panic, but I also wouldn't just guess. Please check with your pharmacist or doctor before adding guggul, especially since your heart medicine is involved.

Here's the gist for these two together: a small human study found that guggul lowered how much diltiazem actually made it into the bloodstream. Diltiazem isn't a prodrug, so this means you may get less of its effect, not more. In plain terms, your blood pressure or chest pain (angina) might not be controlled as well as it should be.

What might that look like? Higher blood pressure readings, or more frequent chest tightness if you take it for angina. The evidence here is limited (one small study), so I wouldn't panic, but I also wouldn't ignore it. Please don't start or stop anything on your own. Check with your pharmacist or doctor first, and if you do use both, it's worth keeping an eye on your blood pressure.

What Is Diltiazem Used For?

Brand names: Cardizem, Cardizem SR, Diltiaz SR, Cardizem CD, Tiazac, Diltiaz XT, Diltiaz CD, Dilacor, Diltiaz, Cardizem LA, Dilacor XR, Cartia XT, Tiamate, Adizem-SR, Adizem-XL, Angitil SR, Angitil XL, Calcicard CR, Dilcardia SR, Dilzem SR, Dilzem XL, Slozem, Tildiem LA, Tildiem Retard, Viazem XL, Zemtard 120XL, Zemtard 180XL, Zemtard 240XL, Zemtard 300XL

Oral extended-release forms of diltiazem — including Matzim LA, Cardizem LA, Cartia, Cartia XT, Tiadylt ER, and various generic capsules and tablets — are used to treat high blood pressure (hypertension) and chronic stable angina (chest pain brought on by activity). Some capsule formulations, including Cartia, Cartia XT, and certain generics, are also approved for angina due to coronary artery spasm (sudden chest pain caused by a sudden tightening of a coronary artery).

Injectable forms of diltiazem are used in hospital settings to temporarily slow a dangerously fast heart rate in atrial fibrillation or atrial flutter (rapid, irregular heart rhythms), and to restore a normal heart rhythm in paroxysmal supraventricular tachycardia (PSVT) — sudden episodes of a very fast but regular heartbeat originating above the heart's lower chambers.
