Simvastatin, Sitagliptin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Guggul interacts with.

Theoretically, guggul might reduce the effects of CYP3A4 substrates.
In vitro research shows that guggul constituents known as guggulsterones can induce CYP3A4.

Juvisync is a combo pill containing simvastatin (a cholesterol-lowering statin) plus sitagliptin (a diabetes medicine). The simvastatin part is the piece that matters here. Your liver clears simvastatin using an enzyme called CYP3A4, and lab studies suggest guggul may speed that enzyme up. If that happens in people, your body could break down simvastatin faster, leaving less of it to do its job. The practical worry is a weaker effect on your cholesterol, not more side effects.

Keep in mind this is only test-tube and animal evidence, so it's theoretical and not proven in patients. Still, since you're treating two conditions at once, it's worth a quick chat with your pharmacist or doctor before adding guggul rather than guessing.
