Sofosbuvir, Velpatasvir, Voxilaprevir falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Guggul interacts with.

Theoretically, guggul might reduce the effects of CYP3A4 substrates.
In vitro research shows that guggul constituents known as guggulsterones can induce CYP3A4.

Here's the concern with adding guggul to Vosevi. Two of the ingredients in Vosevi (velpatasvir and voxilaprevir) are partly cleared by a liver enzyme called CYP3A4. Lab studies suggest the active compounds in guggul, called guggulsterones, may speed that enzyme up. If that happens in people, your body could break down and remove these medicines a little faster, leaving less drug in your blood.

Why does that matter here? Vosevi is fighting a hepatitis C infection, and you usually only get one shot at clearing the virus. Lower drug levels could, in theory, make the treatment less effective. I want to stress this is based on test-tube research only, not human studies, so it's far from proven. Still, since a cure is on the line, I'd skip guggul during your treatment and run it by your pharmacist or doctor first.

What Is Sofosbuvir, Velpatasvir, and Voxilaprevir Used For?

Brand name: Vosevi

Vosevi is used to treat chronic hepatitis C virus (HCV) infection in adults who do not have cirrhosis or who have compensated cirrhosis (mild liver scarring that is still functioning). It is specifically for people who have already been treated with an HCV regimen before and need a different option.

Vosevi is approved for adults with HCV genotype 1, 2, 3, 4, 5, or 6 who previously took a medicine that included an NS5A inhibitor. It is also approved for adults with genotype 1a or 3 who previously took sofosbuvir without an NS5A inhibitor.
