Does it work?
Moderate evidence
Lactobacillus acidophilus La-14 is possibly effective for irritable bowel syndrome (IBS), Helicobacter pylori infection, antibiotic-associated diarrhea, and bacterial vaginosis based on the evidence we hold. It's been rated possibly ineffective for atopic disease (allergic conditions), and the evidence for acute respiratory distress syndrome is insufficient to rate.

How safe is it?
Well-documented data
Lactobacillus acidophilus is generally well tolerated in healthy adults and children. That said, there is some concern about the risk of bacterial infection in certain patients, since live organisms can theoretically move into the bloodstream—cases of bacteremia, sepsis, and other serious infections have been reported, though most are thought to occur in people with underlying conditions.
Orally, this probiotic may cause mild gastrointestinal side effects like stomach discomfort, bloating, flatulence, or diarrhea in some people, but these are uncommon. One trial reported a couple of cases of rash and itching, though it's unclear whether those were due to the probiotic itself.
For pregnancy, the data suggests it's possibly safe; safety data during breastfeeding isn't on file.

Theoretically, taking Lactobacillus acidophilus with antibiotic drugs might decrease the effectiveness of L. acidophilus.
L. acidophilus preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and L. acidophilus preparations by at least two hours.
