Saw palmetto is generally well tolerated, but mild adverse effects like abdominal pain, constipation, diarrhea, dizziness, nausea, and headache occur in some users. Rare cases of heart rhythm problems have been reported.

Pygeum and stinging nettle are both generally well tolerated, though nausea and abdominal pain appear in some research at rates similar to placebo. Stinging nettle can cause constipation or diarrhea.

Beta-sitosterol is generally well tolerated but may cause constipation, diarrhea, gas, indigestion, nausea, or reduced appetite. A rare case of severe pancreatitis was reported.

Acne worsening has been reported with beta-sitosterol alone or combined with saw palmetto. Saw palmetto is rated likely unsafe in pregnancy and unsafe while breastfeeding.
