The combo you're asking about pairs aspirin with codeine. The part that matters here is the aspirin, which thins the blood a bit by keeping platelets from clumping. Saw palmetto has been reported to prolong bleeding time, so stacking the two could, in theory, add up to a slightly higher chance of bruising, nosebleeds, or stomach irritation and bleeding.

I want to be honest about the evidence though: this is based mostly on anecdotal reports, not strong studies, so for most people it's a minor concern. Still, if you take aspirin regularly, notice unusual bruising, or are heading into surgery or a dental procedure, let your pharmacist or doctor know before adding saw palmetto.
