Here's the quick picture: bivalirudin (Angiomax) is a powerful blood thinner you'd only ever get in a hospital, usually during a heart procedure like angioplasty or stenting. Its whole job is to keep your blood from clotting, so the main risk to watch for is bleeding. There are a few scattered reports that saw palmetto may lengthen bleeding time on its own, so the worry is that stacking the two could add up and make bleeding or bruising a little more likely.

That said, this is weak, anecdotal evidence, not something proven in solid studies. And since bivalirudin is given in a closely monitored setting, your care team is already watching your clotting carefully. Just be sure to tell your doctor or pharmacist about any saw palmetto you take so they have the full picture.
