If you take Estrone (Ortho-Est) for things like hot flashes, night sweats, or vaginal dryness, here's the gist: saw palmetto may have mild anti-estrogen activity. In plain terms, it could work against the estrogen you're taking, which might make your symptoms feel less controlled. Estrone is not a prodrug, so it doesn't need activating, the concern is simply that saw palmetto could blunt its effect.

I want to be honest with you though, this is mostly theoretical and comes from weaker studies, so it may not cause real trouble for many people. Still, if you notice your hot flashes or other symptoms creeping back after starting saw palmetto, mention it to your doctor or pharmacist before changing anything.
