Acenocoumarol (Sintrom) is a blood thinner, and like its cousin warfarin it has a narrow margin of safety. Your dose is carefully tuned with INR blood tests, so anything that nudges your bleeding risk can matter. Saw palmetto has been reported, in a few isolated cases, to prolong bleeding time. Paired with acenocoumarol, the concern is added bleeding risk: easy bruising, nosebleeds, bleeding gums, or in rare cases something more serious.

I want to be honest about the evidence here. This is based on anecdotal reports, not strong studies, so for many people it may not cause any real problem. Still, since you are on a drug this sensitive, please check with your doctor or pharmacist before starting saw palmetto, and watch for unusual bleeding.
