The probiotic strains show varied evidence for different uses. Bacillus coagulans is possibly effective for constipation and irritable bowel syndrome (IBS), though evidence for antibiotic-associated diarrhea, athletic performance, cancer support, and fatty liver disease is insufficient.
Bacillus subtilis is possibly effective for antibiotic-associated diarrhea but has insufficient evidence for athletic performance, cirrhosis, constipation, eczema, and diabetes. Saccharomyces boulardii—a beneficial yeast—is possibly effective for rotaviral diarrhea, travelers' diarrhea, antibiotic-associated diarrhea, a serious intestinal condition called necrotizing enterocolitis, C. difficile infection, and H. pylori.
The fermented botanicals (Holy Basil, Broccoli, Astragalus, and Jerusalem Artichoke) lack established evidence for most of their traditional uses in our data.
How safe is it?
Well-documented data
The probiotic strains are generally well tolerated in healthy adults, though safety data and product quality vary widely. B. coagulans and B. subtilis carry a caution: in critically ill or severely immunocompromised patients—including those on central venous catheters, preterm infants, or those with intact immune systems but indwelling medical devices—rare cases of blood infection (bacteremia) and life-threatening infection (sepsis) have been reported.
Similarly, S. boulardii is generally safe for healthy adults but should be avoided by critically ill or immunocompromised patients due to rare reports of yeast blood infection. B. subtilis has rarely caused headache or diarrhea in trials.
Holy Basil most commonly causes loose stools and nausea; the facts note it is possibly unsafe in pregnancy and should be avoided while breastfeeding due to insufficient safety data. Broccoli extract is likely safe in pregnancy but may cause loose stools, diarrhea, abdominal pain, and cramping, especially at high doses.
Astragalus should be avoided in pregnancy and breastfeeding. Jerusalem Artichoke should be avoided during pregnancy and breastfeeding; orally it may cause abdominal pain, diarrhea, flatulence, and nausea.
Acacia Fiber commonly causes abdominal bloating, flatulence, and nausea; these often subside within 2 weeks. Not enough reliable safety data exist for most ingredients overall, so talk with your doctor before using this product.

It may also strengthen the sedative effects of pentobarbital. Broccoli in fermented form may reduce the effectiveness of drugs processed by your liver's CYP1A2 and CYP2A6 enzymes—a Moderate concern.

Acacia Fiber, Fermented
Amoxicillin (Amoxil, Trimox)
Gum arabic can reduce the absorption of amoxicillin.
A small study in healthy volunteers shows that taking amoxicillin and gum arabic concurrently significantly reduces the absorption of amoxicillin. Separate doses of amoxicillin from gum arabic by at least 2 hours.
Oral Drugs
Theoretically, gum arabic can alter the absorption of oral drugs due to its fiber content.
Gum arabic has been used as a suspending osmotic agent in drug formulations. It might improve bioavailability of water-insoluble drugs like naproxen, but reduce absorption of polar drugs like amoxicillin. To avoid changes in absorption, take gum arabic 30-60 minutes after oral medications.
Jerusalem Artichoke, Fermented
Antidiabetes Drugs
Theoretically, artichoke leaf extract may increase the risk of hypoglycemia when taken with antidiabetes drugs.
A meta-analysis of small clinical studies shows that taking artichoke leaf extract for 8-12 weeks can modestly reduce fasting plasma glucose when compared with placebo.
Antihypertensive Drugs
Theoretically, artichoke leaf extract may increase the risk of hypotension when taken with antihypertensive drugs.
A meta-analysis of small clinical studies in patients with hypertension shows that taking artichoke can reduce systolic blood pressure by around 3 mmHg and diastolic blood pressure by around 2 mmHg when compared with placebo.
Cytochrome P450 2B6 (Cyp2B6) Substrates
Theoretically, artichoke might increase serum levels of drugs metabolized by CYP2B6.
In vitro research shows that artichoke leaf extract inhibits CYP2B6 activity. However, this interaction has not been reported in humans.
Cytochrome P450 2C19 (Cyp2C19) Substrates
Theoretically, artichoke might increase serum levels of drugs metabolized by CYP2C19.
In vitro research shows that artichoke leaf extract inhibits CYP2C19 activity. However, this interaction has not been reported in humans.
Holy Basil, Fermented
Anticoagulant/Antiplatelet Drugs
Theoretically, holy basil seed oil might increase the risk of bleeding when used with anticoagulant or antiplatelet drugs.
Animal research shows that holy basil seed oil can prolong bleeding time, possibly due to inhibition of platelet aggregation. However, it is not known if this occurs in humans.
Antidiabetes Drugs
Theoretically, holy basil might increase the risk of hypoglycemia when taken with antidiabetes drugs.
Small clinical studies show that taking holy basil can decrease fasting blood glucose and other measures of glycemic control in patients with type 2 diabetes.
Pentobarbital (Nembutal)
Theoretically, holy basil seed oil might increase the sedative effects of pentobarbital.
Animal research shows that holy basil seed oil increases pentobarbitone-induced sleeping time. However, it is not known if this occurs in humans or if this applies to other barbiturates or sedatives.
Astragalus, Fermented
Antidiabetes Drugs
Theoretically, taking astragalus with antidiabetes drugs might increase the risk of hypoglycemia.
Clinical research in humans shows that astragalus might have hypoglycemic effects. Theoretically, taking astragalus, especially in combination with other hypoglycemic agents, might increase the risk of hypoglycemia.
Cyclophosphamide
Theoretically, astragalus might interfere with cyclophosphamide therapy.
Evidence regarding the effect of astragalus on immunosuppression caused by cyclophosphamide is conflicting. Some animal research suggests that astragalus reverses cyclophosphamide-induced immunosuppression. However, other animal research shows no effect.
Immunosuppressants
Theoretically, astragalus might interfere with immunosuppressive therapy.
Astragalus seems to stimulate immune function. Theoretically, taking astragalus might decrease the effects of immunosuppressive therapy.
Lithium
Theoretically, astragalus might increase levels and adverse effects of lithium.
Animal research suggests that astragalus has diuretic properties. Theoretically, due to this diuretic effect, astragalus might reduce excretion and increase levels of lithium.
Broccoli, Fermented
Cytochrome P450 1A2 (Cyp1A2) Substrates
Theoretically, broccoli might reduce the levels and effects of drugs metabolized by CYP1A2.
Pharmacokinetic research in humans shows that eating 500 grams of fresh broccoli daily for 6-12 days can increase CYP1A2 activity by 10% to 200%. Induction of CYP1A2 activity by broccoli is attributed to its glucosinolate constituents.
Cytochrome P450 2A6 (Cyp2A6) Substrates
Theoretically, broccoli might reduce the levels and effects of drugs metabolized by CYP2A6.
Pharmacokinetic research in humans shows that eating 500 grams of broccoli daily for 6 days increases CYP2A6 activity by 135% to 550%. Induction of CYP2A6 activity is attributed to its glucosinolate constituents.
Probiotic Blend
Antibiotic Drugs
Theoretically, taking Lactobacillus acidophilus with antibiotic drugs might decrease the effectiveness of L. acidophilus.
L. acidophilus preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and L. acidophilus preparations by at least two hours.
