What’s inside
Full disclosure
This product contains one active ingredient: spearmint leaf extract, dry. Spearmint is a common culinary herb; the extract here is a concentrated form.
It's made up with glycerin and water as inactive ingredients—the carriers that keep it in liquid form.
Does it work?
Couldn't assess
The evidence we hold doesn't establish that spearmint works for any of the uses listed here—age-related cognitive decline, brain function, gas, IBS, nausea after surgery, or arthritis. All are rated insufficient reliable evidence, meaning human trials either haven't been done or weren't strong enough to confirm benefit.
That doesn't mean it doesn't work; it means the science isn't settled yet in our data.
How safe is it?
Well-documented data
Spearmint as food or weak tea is generally well tolerated. This concentrated supplement is a different story—the safety data we hold advises caution with supplement amounts unless your doctor signs off.
Allergic reactions are the most common concern: topical contact can cause allergic skin rash or lip inflammation in sensitive people, and rarely, oral spearmint has triggered allergy-related swelling of the soft palate or mouth inflammation. One person in a trial reported anxiety after taking 600 mg daily spearmint extract, though it's unclear whether spearmint caused it.
Pregnancy safety is mixed in our data—rated both Likely Safe and Possibly Unsafe—so talk with your doctor or pharmacist about whether it's right for you if you're pregnant. Lactation appears likely safe based on the data we hold.
Meds to double-check
Moderate interaction found
Check before taking this if you use drugs that damage the liver (hepatotoxic medications) or CNS depressants like sedatives, sleep aids, or anti-anxiety drugs. Both are Moderate-severity concerns.
We hold no interaction data for the inactive ingredients.
The bottom line
Scorecard at a glanceFully disclosed formula with no assessable stated purpose. Moderate medication interactions have been identified, and safety information is well characterized.
This is a single-ingredient spearmint extract. It's generally well tolerated as a food, but as a concentrated supplement it comes with caution flags around liver risk and sedative effects—especially if you take medications for those systems.
If you're pregnant, the safety data is mixed, so check with your doctor or pharmacist before starting. Run your medications through the tool on this page before you take it.

Spearmint Alcohol Free by Hawaii Pharm Drug Interactions
HelloPharmacist Interaction Report
Hawaii Pharm's Spearmint Alcohol Free interacts with two major drug categories through its spearmint leaf extract.
The most serious concern is with drugs that damage the liver (hepatotoxic drugs)—spearmint at high doses may increase that risk, a Moderate-severity interaction. There's also a Moderate-severity interaction with CNS depressants (sedatives, sleep aids, anti-anxiety medications): animal research suggests spearmint's sedative compounds could alter how these drugs work, though human evidence is mixed—spearmint-flavored gum actually made people more alert in one study.
Read the full breakdown — every affected drug type, severity by severity
Altogether, these interactions span 580 individual medications. Because this is a concentrated supplement rather than food or tea, the caution around liver risk and sedative effects applies more strongly than it would to spearmint as a spice or casual tea.
If you take blood pressure medications, sedatives, sleep aids, anxiety drugs, or anything prescribed for liver protection, run your full medication list through the checker on this page before you start.

Spearmint Leaf Extract, Dry
Cns Depressants
Theoretically, spearmint might alter the sedative effects of CNS depressants.
Animal research suggests that (-)-carvone, a major constituent of spearmint, has sedative effects. However, in humans, chewing spearmint-flavored gum induced arousal effects.
Hepatotoxic Drugs
Theoretically, high doses of spearmint might increase the risk of liver damage when taken with hepatotoxic drugs.
Animal research suggests that drinking spearmint tea for 30 days can increase markers of liver damage, including aspartate aminotransferase (AST) and alanine aminotransferase (ALT), and cause liver degeneration and necrosis, in a dose-dependent manner. This effect has not been reported in humans.
