How safe is it?
Well-documented data
Burdock is generally well tolerated as a food and for short-term use in healthy adults, though serious allergic reactions including anaphylaxis are rare. Celery is also well tolerated when taken orally, but allergies — including anaphylaxis — can occur in sensitive people, and it can cause photosensitivity (sun sensitivity).
Hydrangea may cause gastroenteritis, dizziness, and chest tightness. Chanca piedra is often well tolerated short-term, but the most common side effects reported are abdominal pain, nausea, vomiting, and fatigue.
During pregnancy, burdock and hydrangea should be avoided in medicinal amounts due to lack of safety data and possible uterine effects; celery seed in medicinal amounts should also be avoided, though food amounts are likely fine. None of these ingredients have enough safety information for breastfeeding, so they're best avoided while nursing.

The most serious concern is with blood thinners and antiplatelet drugs (medications that prevent clotting): burdock, celery, and chanca piedra (stone breaker) all theoretically increase bleeding risk when combined with these drugs, though this hasn't been confirmed in people.

Celery and chanca piedra can both affect how your body handles lithium — a mood-stabilizing medication — and may raise its levels. Celery also interacts with several other drug types: it can increase levels of venlafaxine (an antidepressant), may reduce how well levothyroxine (a thyroid medication) works, can add to the blood-pressure-lowering effect of antihypertensive drugs, and may interfere with drugs broken down by your liver's CYP1A2 and CYP3A4 enzymes — a broad category that includes many common medications.
Chanca piedra can do similar things with those liver enzymes, and may also reduce the effects of norepinephrine (a blood-pressure medication). Hydrangea raises theoretical lithium concerns as well.
Celery carries a minor interaction risk with photosensitizing drugs and acetaminophen.
Burdock appears to have the fewest documented interactions among the four. Altogether, these interactions span 1,143 individual medications.

Stone Breaker extract
Anticoagulant/Antiplatelet Drugs
Theoretically, chanca piedra might increase the risk of bleeding when used concomitantly with anticoagulant/antiplatelet drugs.
In vitro research suggests that methyl brevifolincarboxylate, a constituent isolated from chanca piedra, can inhibit platelet aggregation. This effect has not been reported in humans.
Cytochrome P450 1A2 (Cyp1A2) Substrates
Theoretically, chanca piedra might reduce the levels and clinical effects of CYP1A2 substrates.
In vitro research shows that chanca piedra extract increases CYP1A2 activity. Theoretically, chanca piedra might increase metabolism of CYP1A2 substrates and lower serum concentrations. This interaction has not been reported in humans.
Cytochrome P450 3A4 (Cyp3A4) Substrates
Theoretically, use of chanca piedra might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that chanca piedra extract inhibits CYP3A4. Theoretically, chanca piedra might increase the levels of CYP3A4 substrates. This interaction has not been reported in humans.
Diuretic Drugs
Theoretically, concomitant use of chanca piedra with diuretics might increase diuresis.
Some preliminary clinical research in adults with hypertension shows that chanca piedra has diuretic properties. However, higher quality research in adults with kidney stones shows taking chanca piedra does not increase urine volume when compared with placebo. Until more is known, use cautiously in patients taking diuretic drugs.
Lithium
Theoretically, chanca piedra might reduce excretion and increase levels of lithium.
Some preliminary clinical research in adults with hypertension shows that chanca piedra has diuretic properties. However, higher quality research in adults with kidney stones shows that taking chanca piedra does not increase urine volume when compared with placebo. Until more is known, use cautiously in patients taking lithium. The dose of lithium might need to be decreased.
Norepinephrine (Levophed)
Theoretically, chanca piedra may reduce the effects of norepinephrine.
Animal research suggests that methyl brevifolincarboxylate, a constituent isolated from chanca piedra, can reverse blood vessel contraction caused by norepinephrine.
Antidiabetes Drugs
Theoretically, concomitant use with antidiabetes drugs might affect glucose control and increase the risk of hypoglycemia.
Animal research suggests that chanca piedra can have hypoglycemic effects. However, a small clinical study in adults with diabetes shows that chanca piedra extract 25 grams orally daily for 1 week does not lower fasting or postprandial blood glucose levels.
Antihypertensive Drugs
Theoretically, concomitant use of chanca piedra with antihypertensive drugs might have additive blood pressure lowering effects.
Animal research suggests that chanca piedra can decrease blood pressure. However, this effect was not observed in most hypertensive patients treated with chanca piedra for 10 days.
Celery extract
Anticoagulant/Antiplatelet Drugs
Theoretically, celery root might increase the risk of bleeding when taken with anticoagulant/antiplatelet drugs.
Celery root contains the constituents falcarinol and falcarindiol. Laboratory research suggests that these constituents can inhibit platelet aggregation. This effect has not been reported in humans.
Antihypertensive Drugs
Theoretically, celery seed extract might have additive effects with antihypertensive drugs.
Clinical research suggests that taking celery seed extract may reduce daytime systolic blood pressure by about 12 mmHg compared to less than 1 mmHg with placebo.
Cytochrome P450 1A2 (Cyp1A2) Substrates
Theoretically, celery might increase levels of drugs metabolized by CYP1A2.
In vitro and animal research suggests that constituents of celery can inhibit CYP1A2. This effect has not been reported in humans.
Levothyroxine (Synthroid, Others)
Theoretically, celery seed might decrease the effects of levothyroxine.
Several cases of hypothyroidism with low T4 levels have been reported in people who were previously stabilized on levothyroxine and then started taking celery seed tablets. They presented with symptoms such as lethargy, bloating, and dry skin, and recovered when celery seed was stopped. However, celery stem and leaf has been associated with case reports of hyperthyroidism in patients with no pre-existing thyroid disorders.
Lithium
Theoretically, celery might reduce excretion and increase levels of lithium due to potential diuretic effects.
Celery is thought to have diuretic properties. However, this effect has not been confirmed in humans.
Venlafaxine (Effexor)
Theoretically, celery root extract might increase blood levels of venlafaxine.
There is one case report of a patient who experienced medication-induced bipolar disorder after beginning to take celery root extract 1000 mg daily along with venlafaxine 75 mg and St. John's wort 600 mg daily. Symptoms included confusion, speech abnormalities, manic affect, and visual hallucinations. The plasma level of venlafaxine was 476.8 ng/mL (normal range 195-400 ng/mL). It is theorized that celery root increased venlafaxine levels by inhibiting cytochrome P450 2D6.
Acetaminophen (Tylenol, Others)
Theoretically, celery juice might increase the effects and side effects of acetaminophen.
Animal research suggests that concomitant use of celery juice plus acetaminophen prolongs the effects of acetaminophen. This effect has been attributed to a decrease in hepatic cytochrome P450 activity. However, other animal research shows that pretreatment with celery root extract protects against acetaminophen-induced acute liver failure. These effects have not been reported in humans.
Photosensitizing Drugs
Theoretically, celery might increase the risk of photosensitivity reactions when taken with photosensitizing drugs.
Laboratory research shows that celery contains photosensitizing agents such as phenols and psoralens.
Burdock extract
Anticoagulant/Antiplatelet Drugs
Theoretically, taking burdock with anticoagulant or antiplatelet drugs might increase the risk of bleeding.
In vitro research shows that lignans from burdock reduce rabbit platelet aggregation by inhibiting platelet activating factor. This interaction has not been reported in humans.
Hydrangea Root Extract
Lithium
Hydrangea is thought to have diuretic properties. Theoretically, due to these potential diuretic effects, hydrangea might reduce excretion and increase levels of lithium. The dose of lithium might need to be decreased.
