If swallowed, symptoms of thujone toxicity can include a rapid or irregular heartbeat, fast breathing, severe stomach upset and vomiting, dilated pupils, and liver or kidney damage — and deaths have been reported from ingestion of very small amounts of tansy oil, sometimes within 1–3.5 hours. Tansy must not be used during pregnancy: it has been used traditionally to bring on menstruation or abortion, and safety data rates it Likely Unsafe.

The same applies to breastfeeding — safety is not established, and thujone content makes it unsafe while nursing. Additionally, contact with tansy or related plants can cause skin reactions like atopic dermatitis, eczema, or photosensitivity (sensitivity to sunlight).
