Tribulus has possibly effective evidence for sexual dysfunction — multiple studies suggest it may help, though the evidence isn't definitive. For athletic performance, the evidence points to possibly ineffective, meaning it's unlikely to boost your workout results.
For benign prostatic hyperplasia (BPH, or enlarged prostate), we don't have enough reliable evidence to say whether it works.
How safe is it?
Well-documented data
Short-term use in healthy adults is generally well tolerated, but we don't have solid long-term safety data. You should avoid tribulus during pregnancy because it may affect hormones and safety hasn't been established.
Avoid it while breastfeeding too, since we don't have reliable information on safety for nursing infants. Gastrointestinal side effects like nausea, cramping, diarrhea, and vomiting can occur, though in one study they happened at similar rates in both tribulus and placebo groups.
Serious effects are rare but have been reported, including liver and kidney injury and seizures; one case involved someone drinking very large amounts of tribulus water daily.

Taking tribulus with antidiabetes drugs might increase the risk of hypoglycemia.
Clinical research shows that Tribulus can lower blood glucose levels in adults with type 2 diabetes who are taking antidiabetes medications.
Antihypertensive Drugs
Theoretically, taking tribulus with antihypertensive drugs might increase the risk of hypotension.
Animal research shows that tribulus can lower blood pressure by inhibiting angiotensin-converting enzyme (ACE). Tribulus has also demonstrated hypotensive effects in pre-hypertensive adults.
Lithium
Theoretically, tribulus might increase the levels and clinical effects of lithium.
Tribulus is thought to have diuretic properties. Due to these potential diuretic effects, tribulus might reduce excretion and increase levels of lithium. The dose of lithium might need to be decreased.
