Moderate evidence
Nopal cactus is rated possibly effective for diabetes based on the evidence we hold. For age-related cognitive decline, benign prostatic hyperplasia (enlarged prostate), indigestion, and high cholesterol (including familial hypercholesterolemia), the evidence is insufficient to rate whether it works.

How safe is it?
Well-documented data
Nopal cactus is generally well tolerated when taken by mouth. The most common side effects are mild: abdominal fullness, indigestion, diarrhea, and nausea.
Rare side effects include headache, insomnia, and dizziness—a clinical study of 41 people saw two with headache, one with dizziness, and one with insomnia. Cases of obstruction in the colon and rectal injury have been reported, but only in people who ate a very large number of prickly pear fruits; these are extremely rare.
Because the dried cactus swells when it absorbs water, it can increase stool volume and frequency. There isn't enough safety data to know whether nopal cactus is safe in supplement amounts during pregnancy or while breastfeeding.
