HelloPharmacist Scorecard of Wild Cherry by Hawaii Pharm
Our pharmacy team’s full take, with four database checks built into the cards below — a summary of what is known, not a grade of the product itself.
What’s inside
Full disclosure
This product contains one active ingredient: certified organic Wild Cherry extract. Wild Cherry has a long history in traditional cough remedies and is commonly found in over-the-counter cough syrups at low doses.
The extract is suspended in vegetable palm glycerin and pharmaceutical-grade alcohol as inactive ingredients.
Does it work?
Couldn't assess
We don't hold effectiveness ratings in our data for Wild Cherry extract. The evidence isn't established in the information available to us, so we can't tell you whether it works for cough or any other purpose based on the facts here.
How safe is it?
Well-documented data
Wild Cherry is generally considered safe in the small amounts used in commercial cough products. However, the plant contains cyanide-forming compounds, and safety hasn't been fully established for this product, especially at higher doses.
Large amounts can lead to cyanide toxicity—a serious poisoning that can be fatal. There's a documented case of a woman who became severely ill and comatose after consuming about 300 grams of wild cherries steeped in alcohol.
Pregnancy safety data rates Wild Cherry as likely unsafe—the product facts advise against use during pregnancy. For breastfeeding, there isn't enough safety information, so avoid use while nursing.
If you're pregnant, planning pregnancy, or breastfeeding, talk with your doctor or pharmacist before taking this product.
Meds to double-check
Moderate interaction found
Before taking this product, check with your doctor or pharmacist if you take any medications metabolized by the liver enzyme CYP3A4—a large group that includes cholesterol drugs like lovastatin and antifungals like ketoconazole. Wild Cherry may raise levels of these drugs, though this hasn't been confirmed in patients yet.
The bottom line
Scorecard at a glanceFully disclosed formula with no assessable stated purpose. Moderate medication interactions have been identified, and safety information is well characterized.
Wild Cherry extract is used in traditional cough remedies and is fine in small commercial amounts, but it can inhibit how your liver processes a broad class of medications. If you take any prescription drugs—especially cholesterol medications or antifungals—check with your doctor or pharmacist before starting.
Avoid it in large doses because of cyanide-forming compounds, and skip it entirely if you're pregnant or breastfeeding.

Wild Cherry by Hawaii Pharm Drug Interactions
HelloPharmacist Interaction Report
Hawaii Pharm's Wild Cherry extract has documented interactions with medications.
The interaction involves CYP3A4 substrates—a large group of drugs your liver metabolizes using a specific enzyme system. In vitro (lab) research suggests wild cherry may inhibit that enzyme, theoretically raising levels of affected drugs in your bloodstream.
However, this interaction hasn't been reported in actual patients yet, so we can't say how often or how severely it matters in real life.
Read the full breakdown — every affected drug type, severity by severity
Drugs metabolized this way include cholesterol medications like lovastatin, antifungals like ketoconazole and itraconazole, and many others. If you take any of these or similar medications, it's worth a conversation with your doctor or pharmacist before starting this product.
Altogether, these interactions span 644 individual medications.
Beyond drug interactions, there's a separate safety concern with wild cherry itself: the plant contains cyanide-forming compounds. In large amounts, it can cause cyanide toxicity, which is serious.
This product should only be used in the small amounts typical of commercial cough formulas—never in large quantities.

Cytochrome P450 3A4 (Cyp3A4) Substrates
In vitro research suggests that wild cherry can inhibit cytochrome P450 3A4 (CYP3A4) enzymes. Theoretically, wild cherry might increase levels of drugs metabolized by CYP3A4. However, so far, this interaction has not been reported in humans.
Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and others.
