The dose of dexamethasone must be reduced when used with netupitant because concomitant use significantly increases the exposure of dexamethasone due to inhibition of CYP3A4-mediated dexamethasone metabolism. In a pharmacokinetic study, a 2-fold increase in dexamethasone exposure persisted for 4 days after a single netupitant dose. After netupitant 300 mg/palonosetron 0.5 mg was coadministered with dexamethasone in another study, dexamethasone exposure increased by up to 2.4-fold on day 41.
