Increased plasma concentrations of paricalcitol


Interaction Summary

Paricalcitol is a substrate for cytochrome P450 3A. Coadministration of paricalcitol with strong CYP3A inhibitors, such as ketoconazole, should be approached with caution. Monitor closely iPTH and serum calcium concentrations when initiating or discontinuing ketoconazole therapy, and adjust paricalcitol dosage when necessary.

How To Manage Interaction

Concomitant administration of paricalcitol and ketoconazole may increase levels of paricalcitol, resulting in oversuppression of parathyroid hormone (PTH). Monitor iPTH and serum calcium concentrations. Dosage adjustment of paricalcitol may be necessary.

Literature Reports

A) In healthy subjects, coadministration of ketoconazole (200 milligrams twice daily for 5 days) and paricalcitol increased the AUC and mean half-life of paricalcitol. The paricalcitol AUC was approximately doubled, and mean half-life increased from 9.8 hours with paricalcitol alone to 17 hours in the presence of ketoconazole .

Ketoconazole Overview

  • Ketoconazole is used to treat fungal infections when other medications are not available or cannot be tolerated. Ketoconazole should not be used to treat fungal meningitis (infection of the membranes surrounding the brain and spinal cord caused by a fungus) or fungal nail infections. Ketoconazole is in a class of antifungals called imidazoles. It works by slowing the growth of fungi that cause infection.
