What happens

An increased risk of bleeding

Coadministration of zanubrutinib with meloxicam (an antiplatelet agent) may further increase the risk of hemorrhage; bleeding has occurred in patients with and without concomitant anticoagulant or antiplatelet therapy3. Use of NSAIDs, including meloxicam, may increase the risk of bleeding events, including gastrointestinal bleeding 21. Monitor for bleeding if meloxicam is used concomitantly with drugs that interfere with hemostasis 21. Discontinue zanubrutinib if intracranial hemorrhage of any grade occurs. Consider the benefit-risk of withholding zanubrutinib for 3-7 days before and after surgery depending upon the type of surgery and the risk of bleeding 3.

a) In a systematic review and metaanalysis of 10 studies (including 7 case-control and 3 cohort studies; N=1,489,120 participants), non-aspirin NSAID use was associated with a small, non-significant risk of hemorrhagic stroke as a single group (pooled RR, 1.09; 95% CI, 0.98 to 1.22). In an analysis of individual NSAIDs, there was a small, but significant increased risk of hemorrhagic stroke with meloxicam (RR, 1.27; 95% CI, 1.08 to 1.5) or diclofenac (RR, 1.27; 95% CI, 1.02 to 1.59). Several limitations to the included studies, such as reliance on coding-based medical registries or prescription databases, potential for recall bias, inconsistent adjustment for confounders, and the generally older population included may influence the generalizability of the results 4.
