Zanubrutinib falls under CYTOCHROME P450 3A4 (CYP3A4) INDUCERS — the group Alkanna interacts with.

Hepatotoxic pyrrolizidine alkaloids (PAs) are substrates of cytochrome P450 3A4 (CYP3A4). Theoretically, drugs that induce CYP3A4 might increase the conversion of PAs to toxic metabolites. Some drugs that induce CYP3A4 include carbamazepine (Tegretol), phenobarbital, phenytoin (Dilantin), rifampin, rifabutin (Mycobutin), and others.

Here's the angle that matters most with this pair. Alkanna can contain liver-toxic compounds called pyrrolizidine alkaloids, and your liver uses an enzyme called CYP3A4 to turn those compounds into harmful, liver-damaging byproducts. The concern on file is that medications which speed up (induce) CYP3A4 could ramp up that conversion and increase the strain on your liver from Alkanna.

Now, a quick reality check for Zanubrutinib (Brukinsa): it is mainly broken down by CYP3A4, but it is not a strong inducer of it, so its fit in this category is loose and the worry is largely theoretical. Still, both your cancer and zanubrutinib itself can already affect the liver and blood counts, so adding a plant with known liver toxicity is not worth the risk. Please skip oral Alkanna and run any use past your oncology pharmacist first.

Alkanna is a Mediterranean plant whose red root has long been used as a dye and as a folk skin remedy for wounds and burns. Human evidence for any health benefit is very limited, and because the plant can contain liver-toxic pyrrolizidine alkaloids, oral use is not recommended. What Is Zanubrutinib Used For?

Brand name: Brukinsa

Brukinsa (zanubrutinib) is used to treat adult patients with several blood cancers. These include chronic lymphocytic leukemia (CLL), small lymphocytic lymphoma (SLL), Waldenström's macroglobulinemia (WM), mantle cell lymphoma (MCL) after at least one prior treatment, relapsed or refractory marginal zone lymphoma (MZL) after at least one prior anti-CD20 regimen, and relapsed or refractory follicular lymphoma (FL) in combination with obinutuzumab after two or more prior lines of treatment.

Some of these uses — MCL, MZL, and FL — are approved under accelerated approval, meaning they are based on tumor response rates rather than confirmed long-term survival benefit. Continued approval for those uses may depend on results from confirmatory trials.
