Glutethimide falls under CYTOCHROME P450 3A4 (CYP3A4) INDUCERS — the group Alkanna interacts with.

Hepatotoxic pyrrolizidine alkaloids (PAs) are substrates of cytochrome P450 3A4 (CYP3A4). Theoretically, drugs that induce CYP3A4 might increase the conversion of PAs to toxic metabolites. Some drugs that induce CYP3A4 include carbamazepine (Tegretol), phenobarbital, phenytoin (Dilantin), rifampin, rifabutin (Mycobutin), and others.

Here is the thing to know about pairing alkanna with glutethimide (Doriden). Glutethimide is an old sedative sleep medicine, and it speeds up a liver enzyme called CYP3A4. That same enzyme is what turns the pyrrolizidine alkaloids in alkanna root into harmful, liver-toxic byproducts. So in theory, taking glutethimide could make those alkaloids more dangerous to your liver, not the other way around. The concern here is really about the supplement becoming more toxic, not about changing how your sleep medicine works.

To be fair, this is a theoretical worry based on how the chemistry works, not on real patient studies. Still, oral alkanna is not recommended in the first place because of those liver-toxic compounds. Please check with your pharmacist or doctor before using any alkanna product.
