Cenobamate falls under CYTOCHROME P450 3A4 (CYP3A4) INDUCERS — the group Alkanna interacts with.

Hepatotoxic pyrrolizidine alkaloids (PAs) are substrates of cytochrome P450 3A4 (CYP3A4). Theoretically, drugs that induce CYP3A4 might increase the conversion of PAs to toxic metabolites. Some drugs that induce CYP3A4 include carbamazepine (Tegretol), phenobarbital, phenytoin (Dilantin), rifampin, rifabutin (Mycobutin), and others.

Here's the key thing about this pairing: cenobamate (Xcopri) speeds up an enzyme in your liver called CYP3A4. Alkanna root naturally contains pyrrolizidine alkaloids, which are liver-toxic compounds. Those compounds get processed by that same enzyme, and that processing is actually what turns them into their harmful, liver-damaging forms. So by revving up CYP3A4, cenobamate could theoretically push more of the alkanna toxins into their toxic version, raising the strain on your liver.

I'll be honest, this is a theory based on how the chemistry works, not on real patient cases, so I don't want to alarm you. But alkanna is risky to swallow on its own. The safest move is to skip oral alkanna entirely and only consider skin use after checking with your pharmacist or doctor. Never adjust your cenobamate.
