Rufinamide falls under CYTOCHROME P450 3A4 (CYP3A4) INDUCERS — the group Alkanna interacts with.

Hepatotoxic pyrrolizidine alkaloids (PAs) are substrates of cytochrome P450 3A4 (CYP3A4). Theoretically, drugs that induce CYP3A4 might increase the conversion of PAs to toxic metabolites. Some drugs that induce CYP3A4 include carbamazepine (Tegretol), phenobarbital, phenytoin (Dilantin), rifampin, rifabutin (Mycobutin), and others.

Here's the concern with pairing oral Alkanna and Rufinamide (Banzel). Alkanna root can contain pyrrolizidine alkaloids, and the liver has to process those compounds using an enzyme called CYP3A4. Rufinamide mildly speeds that enzyme up. In theory, a faster enzyme could turn more of those plant alkaloids into harmful byproducts, which may raise the risk of liver injury. So the worry here is mostly about the supplement becoming more toxic, not about your seizure control changing.

I'll be honest: this is a theoretical interaction based on how things work in the lab, not real-world reports. Still, oral Alkanna isn't recommended anyway because of those liver-toxic compounds. If you only use it on your skin, swallowing isn't the issue. Please loop in your pharmacist or doctor before using any Alkanna while you're on Rufinamide.

Rufinamide — available as Banzel and generic rufinamide — is used as an add-on treatment (alongside other seizure medicines) for seizures associated with Lennox-Gastaut Syndrome (LGS), a severe type of epilepsy that usually begins in childhood and causes multiple types of seizures.

This indication applies to both the tablet and oral suspension forms, for pediatric patients 1 year of age and older as well as adults.
