Rifampin falls under CYTOCHROME P450 3A4 (CYP3A4) INDUCERS — the group Alkanna interacts with.

Hepatotoxic pyrrolizidine alkaloids (PAs) are substrates of cytochrome P450 3A4 (CYP3A4). Theoretically, drugs that induce CYP3A4 might increase the conversion of PAs to toxic metabolites. Some drugs that induce CYP3A4 include carbamazepine (Tegretol), phenobarbital, phenytoin (Dilantin), rifampin, rifabutin (Mycobutin), and others.

Here's the concern with this pairing. Alkanna root can contain pyrrolizidine alkaloids, which are compounds that can harm the liver. Your body uses a liver enzyme called CYP3A4 to process them, and that processing can turn them into more toxic byproducts. Rifampin is a strong booster of CYP3A4, meaning it revs up that enzyme. In theory, that could speed up the creation of those harmful liver byproducts from Alkanna.

I'll be honest: this is a theoretical worry based on how the enzyme works, not on real patient cases, so don't panic. But rifampin already puts stress on the liver on its own, and oral Alkanna isn't recommended anyway. Please skip swallowing Alkanna while on rifampin, and check with your pharmacist or doctor before using it even on your skin.

Alkanna is a Mediterranean plant whose red root has long been used as a dye and as a folk skin remedy for wounds and burns. Human evidence for any health benefit is very limited, and because the plant can contain liver-toxic pyrrolizidine alkaloids, oral use is not recommended. If you use it at all, use it externally and check with a pharmacist or doctor first.

About Rifampin

What Is Rifampin Used For?

Brand names: Rifadin, Rimactane, Rifadin Injection, Rofactor

Rifampin treats all forms of tuberculosis (TB). It is almost always used in combination with other antibiotics — your doctor will build a multi-drug regimen tailored to your infection. Treatment usually runs for at least 6 months total.

Rifampin is also used to clear Neisseria meningitidis bacteria from the nose and throat in people who carry the germ without being sick themselves. It is not used to treat active meningococcal disease. The intravenous (IV) forms — Rifadin IV, Rifadin IV NOVAPLUS, and the injectable form of Rifampin — are specifically indicated when a patient cannot take the drug by mouth.
