Enzalutamide falls under CYTOCHROME P450 3A4 (CYP3A4) INDUCERS — the group Alkanna interacts with.

Hepatotoxic pyrrolizidine alkaloids (PAs) are substrates of cytochrome P450 3A4 (CYP3A4). Theoretically, drugs that induce CYP3A4 might increase the conversion of PAs to toxic metabolites. Some drugs that induce CYP3A4 include carbamazepine (Tegretol), phenobarbital, phenytoin (Dilantin), rifampin, rifabutin (Mycobutin), and others.

Here's the concern with pairing Alkanna and enzalutamide (Xtandi). Enzalutamide is a strong inducer of a liver enzyme called CYP3A4, meaning it revs that enzyme up. Alkanna root can contain pyrrolizidine alkaloids, and those compounds get processed by that same CYP3A4 pathway into toxic, liver-damaging byproducts. So in theory, taking enzalutamide alongside oral Alkanna could speed up the formation of those harmful metabolites and raise the risk of liver injury.

I want to be honest: this is a theoretical interaction based on how the chemicals behave, not on real patient cases. Still, oral Alkanna already isn't recommended because of liver toxicity. Using it on the skin is far less of a worry. Please run any Alkanna use past your pharmacist or oncologist first.

Alkanna is a Mediterranean plant whose red root has long been used as a dye and as a folk skin remedy for wounds and burns. Human evidence for any health benefit is very limited, and because the plant can contain liver-toxic pyrrolizidine alkaloids, oral use is not recommended. If you use it at all, use it externally and check with a pharmacist or doctor first.

About Enzalutamide

What Is Enzalutamide Used For?

Brand name: Xtandi

Xtandi (enzalutamide) is used to treat prostate cancer in three specific situations: castration-resistant prostate cancer (CRPC), metastatic castration-sensitive prostate cancer (mCSPC), and non-metastatic castration-sensitive prostate cancer with biochemical recurrence at high risk for metastasis (high-risk BCR).

Castration-resistant means the cancer has continued to grow despite low testosterone levels. Castration-sensitive means the cancer still responds to testosterone-lowering therapy. In most of these cases, Xtandi is used alongside a GnRH analog (a hormone-lowering injection) or after surgical removal of the testicles — except in the high-risk BCR setting, where it may be used with or without a GnRH analog.
