Lorlatinib falls under CYTOCHROME P450 3A4 (CYP3A4) INDUCERS — the group Alkanna interacts with.

Hepatotoxic pyrrolizidine alkaloids (PAs) are substrates of cytochrome P450 3A4 (CYP3A4). Theoretically, drugs that induce CYP3A4 might increase the conversion of PAs to toxic metabolites. Some drugs that induce CYP3A4 include carbamazepine (Tegretol), phenobarbital, phenytoin (Dilantin), rifampin, rifabutin (Mycobutin), and others.

Here's the angle that matters with this pair. Alkanna root can contain pyrrolizidine alkaloids, which are compounds your liver can turn into toxic byproducts using the CYP3A4 enzyme. Lorlatinib (Lorbrena) is a mild CYP3A4 inducer, meaning it can rev up that enzyme. In theory, that faster enzyme activity could convert more of Alkanna's alkaloids into the harmful, liver-stressing forms. So the worry here is added strain on the liver, not a change in how well your cancer treatment works.

I'll be honest, this is a theoretical concern based on how these chemicals behave, not on real patient cases. Even so, oral Alkanna isn't recommended on its own because of its liver risk, and lorlatinib already requires liver monitoring. Please don't take Alkanna by mouth while on Lorbrena without clearing it with your oncology team first.

Alkanna is a Mediterranean plant whose red root has long been used as a dye and as a folk skin remedy for wounds and burns. What Is Lorlatinib Used For?

Lorbrena (lorlatinib) is used to treat adults with metastatic non-small cell lung cancer (NSCLC) — meaning lung cancer that has spread — when lab testing confirms the tumor is ALK-positive (has a specific gene change that drives cancer growth). Patients must be tested with an FDA-approved diagnostic test before starting treatment.

Lorbrena has been studied in patients who have not yet received treatment for advanced disease, as well as in patients whose cancer progressed on other ALK-targeting medicines.
