What happens

Increased miFEPRIStone exposure and an increased asciminib exposure

Interaction Deep Dive

Concomitant use of asciminib (CYP3A4 substrate) with miFEPRIStone (strong CYP3A4 inhibitor) may increase asciminib exposure, which may increase the risk of asciminib-related adverse reactions. Closely monitor patients for adverse reactions in patients treated with asciminib at 200 mg twice daily with concomitant use of miFEPRIStone1. Use extreme caution when miFEPRIStone is coadministered with drugs that have high first pass metabolism via CYP3A4. Use the lowest dose and/or decreased frequency with therapeutic monitoring. Prefer alternative drugs without these metabolic characteristics when possible. Coadministration of miFEPRIStone (CYP3A4 substrate) and asciminib (CYP3A4 inhibitor) may also increase miFEPRIStone exposure. Carefully weigh the benefit of concomitant use of these agents against the potential risks. Limit the dose of miFEPRIStone to 900 mg. Additionally, wait at least 2 weeks after stopping miFEPRIStone before initiating asciminib 2.

a) The asciminib AUC (inf) and Cmax increased by 36% and 19%, respectively, following coadministration of a single asciminib dose of 40 mg with a strong CYP3A4 inhibitor (clarithromycin) 1.

b) In a pharmacokinetic study, the concomitant administration of a single oral dose of ALPRAZolam 1 mg in healthy subjects receiving miFEPRIStone 1200 mg once daily for 10 days resulted in increased ALPRAZolam (CYP3A4 inhibitor) exposure. Compared with ALPRAZolam administration alone, coadministration with miFEPRIStone resulted in an AUC and Cmax geometric mean ratio of 1.8 and 0.81, respectively for ALPRAZolam, and 0.76 and 0.39, respectively for the 4-hydroxy-ALPRAZolam metabolite 2.
