Treprostinil falls under ANTIHYPERTENSIVE DRUGS — the group Algal Oil interacts with.
Algal oil usually contains docosahexaenoic acid (DHA) and/or eicosapentaenoic acid (EPA). There is evidence that fish oil, which also contains DHA and EPA, can modestly lower blood pressure and might have additive effects in patients treated with antihypertensive drugs.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the thing to know about pairing algal oil with treprostinil (Orenitram). Treprostinil already relaxes and widens blood vessels to treat pulmonary hypertension, and one of its built-in side effects is lowering blood pressure. Omega-3 fats like the DHA and EPA in algal oil can nudge blood pressure down a little too. So in theory, taking both could add up and drop your pressure more than expected.
I'll be honest, this is a theoretical concern based on how these work, not something proven to cause problems. Watch for lightheadedness, dizziness, flushing, or headache, especially when standing up. Food amounts of omega-3s are unlikely to matter, but if you're taking a concentrated daily supplement, mention it to your pharmacist or doctor first.

The risk of interaction is highest for algal oil containing high amounts (> 2 grams daily) of eicosapentaenoic acid (EPA), or EPA and docosahexaenoic acid (DHA). High doses of EPA or oils containing EPA and DHA can reduce platelet aggregation in humans. However, most algal oil contains very little EPA and larger amounts of DHA. While some conflicting evidence exist, most research shows that DHA alone does not affect blood clotting.
Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..
Here's the connection: treprostinil (Orenitram) not only widens blood vessels in the lungs but also makes platelets a little less sticky, so it can mildly raise bleeding risk on its own. Algal oil's omega-3 fats can do something similar in high doses, so stacking the two could, in theory, add up to a bit more bruising or bleeding.
The good news is this concern is mostly theoretical and rated minor and unlikely. Most algal oil is rich in DHA with very little EPA, and DHA alone generally doesn't affect clotting much. The risk mainly climbs with concentrated EPA-containing products over about 2 grams a day. Watch for easy bruising or nosebleeds, and check with your pharmacist or doctor before adding it.

Algal oil is a plant-based source of the omega-3 fats DHA and (in some products) EPA, making it a popular alternative to fish oil for vegetarians, vegans, and people who dislike fish. There is reasonable evidence that omega-3s can lower triglycerides and support heart and brain health, though more research is needed for many specific uses. It is generally well tolerated, but check with your pharmacist or doctor before starting, especially if you take blood thinners or are pregnant.
About Treprostinil
What Is Treprostinil Used For?
Treprostinil treats pulmonary arterial hypertension (PAH; WHO Group 1) — high blood pressure in the arteries of the lungs. Depending on the product and route, it is used to improve exercise ability, delay disease progression, or reduce symptoms tied to physical activity. Remodulin and treprostinil injection products are also indicated for patients who need to transition away from another PAH medicine called epoprostenol.
Tyvaso, Tyvaso DPI, and Yutrepia (inhaled forms) are additionally approved to treat pulmonary hypertension associated with interstitial lung disease (PH-ILD; WHO Group 3) — lung-pressure problems caused by scarring or inflammation in the lung tissue itself. Orenitram (extended-release oral tablet) is specifically indicated to delay disease progression and improve exercise capacity in PAH.
