Iloprost falls under ANTIHYPERTENSIVE DRUGS — the group Algal Oil interacts with.
Algal oil usually contains docosahexaenoic acid (DHA) and/or eicosapentaenoic acid (EPA). There is evidence that fish oil, which also contains DHA and EPA, can modestly lower blood pressure and might have additive effects in patients treated with antihypertensive drugs.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the simple version: iloprost (Ventavis) is a vasodilator, meaning it relaxes and widens your blood vessels to ease the pressure in your lungs. Algal oil's omega-3s (DHA and EPA) may gently lower blood pressure on their own. Put the two together and the blood-pressure-lowering effects could stack up a bit. That might leave you feeling more lightheaded, dizzy, flushed, or faint, especially when you stand up quickly.
I want to be honest about the strength of this: it's theoretical, based on how these work rather than reports of real problems, and any effect from algal oil tends to be mild. Still, since PAH is serious, don't change anything on your own. Just give your pharmacist or doctor a heads-up before adding it so they can keep an eye on your blood pressure.

The risk of interaction is highest for algal oil containing high amounts (> 2 grams daily) of eicosapentaenoic acid (EPA), or EPA and docosahexaenoic acid (DHA). High doses of EPA or oils containing EPA and DHA can reduce platelet aggregation in humans. However, most algal oil contains very little EPA and larger amounts of DHA. While some conflicting evidence exist, most research shows that DHA alone does not affect blood clotting.

Here's the thing with iloprost: besides relaxing the blood vessels in your lungs, it also makes your platelets a little less sticky, so it gently nudges you toward easier bleeding on its own. Algal oil's omega-3s (mainly DHA, with some EPA in certain products) can do something similar at higher doses. Put them together and, in theory, you could bruise more easily or notice a nosebleed or gums that bleed when you brush.
In real life this is a minor, unlikely concern. Most algal oil is mostly DHA, and DHA alone doesn't seem to meaningfully affect clotting. It matters more if your product packs over 2 grams of EPA daily. Still, it's worth a quick word with your pharmacist or doctor before adding it.

Specifically, it is used to reduce the risk of losing fingers or toes (digit amputation) after serious frostbite injury. The clinical evidence supporting this use comes from young, healthy adults who suffered frostbite at high altitudes.
