Aspirin, Codeine Phosphate falls under NEPHROTOXIC DRUGS — the group Aristolochia interacts with.

Aristolochia is nephrotoxic. There are numerous cases of nephropathy and renal failure associated with aristolochia use. Theoretically, combining aristolochia with potentially nephrotoxic drugs might have additive adverse effects on kidney function. However, this interaction has not yet been reported in humans. Close monitoring of renal function in patients taking aristolochia with nephrotoxic drugs may be warranted.
Some potentially nephrotoxic drugs include cyclosporine (Neoral, Sandimmune); aminoglycosides including amikacin (Amikin), gentamicin (Garamycin, Gentak, others), and tobramycin (Nebcin, others); nonsteroidal anti-inflammatory drugs (NSAIDs) including ibuprofen (Advil, Motrin, Nuprin, others), indomethacin (Indocin), naproxen (Aleve, Anaprox, Naprelan, Naprosyn), piroxicam (Feldene); and numerous others.

Let me be upfront: Aristolochia is a supplement I would steer anyone away from entirely. It contains aristolochic acid, which has been clearly linked to serious kidney damage and even cancer. That concern stands on its own, no matter what else you take.

Now, the specific worry here is the aspirin part of your Aspirin with Codeine. Aspirin, like other anti-inflammatory pain relievers, can be hard on the kidneys, especially with regular use or if you're dehydrated. Pairing it with a known kidney toxin like Aristolochia could, in theory, add up to more strain on those filtering organs. This is a theoretical concern based on how these substances work, not something proven in people taking them together. The codeine side isn't the issue here. Please don't change your prescription, but do talk with your pharmacist or doctor, and skip the Aristolochia.
