Guanethidine, Hydrochlorothiazide falls under NEPHROTOXIC DRUGS — the group Aristolochia interacts with.

Aristolochia is nephrotoxic. There are numerous cases of nephropathy and renal failure associated with aristolochia use. Theoretically, combining aristolochia with potentially nephrotoxic drugs might have additive adverse effects on kidney function. However, this interaction has not yet been reported in humans. Close monitoring of renal function in patients taking aristolochia with nephrotoxic drugs may be warranted.
Some potentially nephrotoxic drugs include cyclosporine (Neoral, Sandimmune); aminoglycosides including amikacin (Amikin), gentamicin (Garamycin, Gentak, others), and tobramycin (Nebcin, others); nonsteroidal anti-inflammatory drugs (NSAIDs) including ibuprofen (Advil, Motrin, Nuprin, others), indomethacin (Indocin), naproxen (Aleve, Anaprox, Naprelan, Naprosyn), piroxicam (Feldene); and numerous others.

Here's the honest first thing I want you to know: Aristolochia is not safe for anyone to take. It contains aristolochic acid, which is directly toxic to the kidneys and is linked to permanent DNA damage and cancer. The FDA has warned against any product containing it, so this isn't really about fine-tuning a dose.

On top of that, your Esimil contains hydrochlorothiazide, a water pill that already leans on your kidneys to work and can shift your fluids and salts. Pairing a kidney toxin with a medicine that stresses the kidneys could, in theory, add up to more harm, things like reduced urine output, swelling, or worsening kidney numbers on bloodwork. This combined concern is based on pharmacology, not proven human reports, but please don't use Aristolochia. Talk with your pharmacist or doctor.
