Bacillus Coagulans and Cefpodoxime Proxetil

Here is the interaction we found between these two, with the evidence behind it.

Moderate

ANTIBIOTIC DRUGS

Why this applies to Cefpodoxime Proxetil

Cefpodoxime Proxetil falls under ANTIBIOTIC DRUGS — the group Bacillus Coagulans interacts with.

Theoretically, taking antibiotics with Bacillus coagulans might decrease the effectiveness of B. coagulans.
B. coagulans preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and B. coagulans preparations by at least two hours.

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the simple version: cefpodoxime is an antibiotic, and its whole job is to kill bacteria. Bacillus coagulans is a beneficial bacteria (a probiotic). So if you swallow them at the same time, the antibiotic may wipe out a good chunk of those live probiotic organisms before they can do their work. This doesn't make your infection harder to treat, and it won't hurt you. It just means you may not get the full digestive benefit you were hoping for from the probiotic.

The easy fix is timing. Try to space them out by at least two hours. Keep finishing your full cefpodoxime course as prescribed, and if you're unsure how to schedule them, your pharmacist can help you map it out. This is a theoretical concern based mostly on lab reasoning, so don't stress over it.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

From our pharmacist’s overview on the Cefpodoxime Proxetil page

“If you have been prescribed cefpodoxime proxetil (Vantin), an oral antibiotic, this list calls for genuine care, and activated charcoal is where to start. It is the only major-rated interaction among the 55 on file, and it can soak up the antibiotic in your gut, leaving you with less medicine than your infection needs.”
