Hydrochlorothiazide, Lisinopril falls under ORGANIC ANION TRANSPORTER 1 (OAT1) SUBSTRATES — the group Banderol interacts with.

Small lab studies have found that Banderol may inhibit OATP1B1 (Organic Anion Transporting Polypeptide 1B1), which is a membrane protein responsible for the uptake of various drugs, hormones, and other biologically active substances into the cells of the liver and other tissues. It is often referred to as a drug transporter. It is a member of the OATP family of transporters and is known to play a critical role in the pharmacokinetics of many drugs. When OATPs are inhibited, it can cause problems with the elimination of drugs and result in a sudden rise in the concentration of the drug in the bloodstream and the extent to which it is absorbed (AUC) for drugs that rely on these transporters. This can increase the likelihood of adverse side effects associated with the therapy.

Here's the gist for your Zestoretic (lisinopril with hydrochlorothiazide). Banderol has shown in test-tube studies that it can block a liver transporter called OATP1B1, which helps move certain drugs into cells so the body can clear them. If that transporter is slowed down, a drug that depends on it can build up in the blood, and stronger levels can mean more side effects.

For this combo, that could look like extra dizziness, lightheadedness when standing, low blood pressure, or more pronounced fluid and salt shifts. Honestly though, this is based on lab data, not real-world human studies, so it's more of a theoretical caution than a proven problem. Still, blood pressure meds are worth being careful with. Small lab studies have found that Banderol may also inhibit OATP1B3 (Organic Anion Transporting Polypeptide 1B3), in addition to OATP1B1. If a drug is a substrate for both transporters, the risk of interaction with Banderol increases but the overall clinical significance is not well understood.

Here's the short version on combining Banderol with your Zestoretic (lisinopril plus hydrochlorothiazide). The concern on file is that Banderol might block certain transporter proteins (called OATP and related ones) that help move some medications around the body. In theory, if your blood pressure pill leans on those same transporters, Banderol could slow its clearance and let a bit more build up. That might mean a slightly bigger blood-pressure-lowering effect, so you'd watch for dizziness, lightheadedness when standing, or feeling unusually tired.

I want to be honest though: this comes only from small test-tube studies, and the real-world importance isn't well understood. It's worth mentioning to your pharmacist or doctor, but it's not a reason to panic or stop anything on your own.

What Is Lisinopril and Hydrochlorothiazide Used For?

Brand names: Zestoretic, Prinzide

Lisinopril and hydrochlorothiazide tablets — sold as the brand Zestoretic and as the generic combination — are used to treat high blood pressure (hypertension). Lowering blood pressure reduces the risk of serious cardiovascular events such as stroke and heart attack.

This is a fixed-dose combination product and is not intended as a first-line (initial) treatment for high blood pressure. It is typically prescribed for patients whose blood pressure is not adequately controlled on a single medication alone.
