Brexpiprazole falls under DOPAMINE AGONISTS — the group Black Horehound interacts with.

Theoretically, black horehound might have additive effects when used with dopamine agonists. Some constituents of black horehound bind to dopamine D2 receptors in vitro; however, this has not yet been reported in humans. Some dopamine agonists include bromocriptine (Parlodel), levodopa (Dopar, component of Sinemet), pramipexole (Mirapex), ropinirole (Requip), and others.

Here's a quick heads-up about pairing black horehound with brexpiprazole (Rexulti). The concern is mostly theoretical. In test-tube studies, some compounds in black horehound seem to grab onto dopamine D2 receptors in the brain. That's interesting because brexpiprazole works partly by acting on those very same dopamine receptors.

So in theory, the two could tug at the same system and step on each other's effects, which might blunt how well your medication controls symptoms, or possibly add to side effects like restlessness, drowsiness, or movement changes. I want to be honest, though: this has only been seen in lab dishes, never reported in real people, so the actual risk is unclear. It's not a reason to panic. Just loop in your pharmacist or doctor before adding the herb so they can keep an eye on things.

Black horehound is a traditional European herb mainly used for nausea, mild anxiety, and digestive upset, but high-quality human studies are lacking, so its effectiveness is largely unproven. It has a strong, unpleasant smell and taste, and its safety has not been well studied, so it should be avoided in pregnancy and breastfeeding and used cautiously by everyone else.

What Is Brexpiprazole Used For?

Brand name: Rexulti

Brexpiprazole tablets are approved for two uses in adults: as an add-on (adjunctive) treatment alongside antidepressants for major depressive disorder (MDD), and as a treatment for schizophrenia. These uses apply to both the generic brexpiprazole tablets and Rexulti.

Rexulti has one additional approved use: treating agitation associated with dementia due to Alzheimer's disease in adults. Rexulti is also approved for schizophrenia in adolescents ages 13 and older. Note that Rexulti is not approved for dementia-related psychosis without agitation from Alzheimer's disease, and it is not meant to be used on an as-needed basis for Alzheimer's-related agitation.
