Tolcapone falls under DOPAMINE AGONISTS — the group Black Horehound interacts with.

Theoretically, black horehound might have additive effects when used with dopamine agonists. Some constituents of black horehound bind to dopamine D2 receptors in vitro; however, this has not yet been reported in humans. Some dopamine agonists include bromocriptine (Parlodel), levodopa (Dopar, component of Sinemet), pramipexole (Mirapex), ropinirole (Requip), and others.

Here's the honest picture with tolcapone (Tasmar) and black horehound. Tolcapone itself isn't a dopamine agonist. It works behind the scenes by blocking an enzyme called COMT, which keeps your levodopa working longer so your Parkinson's medicine stays effective between doses. Because it boosts dopamine activity, it gets grouped with these other medicines for interaction purposes.

The concern on file is purely theoretical. In lab dishes, certain compounds in black horehound touch dopamine D2 receptors, so in theory they might add to dopamine-related effects like nausea, dizziness, low blood pressure, or extra involuntary movements. This has never actually been shown in people, so I wouldn't lose sleep over it. Still, since tolcapone needs careful liver monitoring, please run any herbal supplement past your doctor or pharmacist first.

Black horehound is a traditional European herb mainly used for nausea, mild anxiety, and digestive upset, but high-quality human studies are lacking, so its effectiveness is largely unproven. It has a strong, unpleasant smell and taste, and its safety has not been well studied, so it should be avoided in pregnancy and breastfeeding and used cautiously by everyone else.

What Is Tolcapone Used For?

Brand name: Tasmar

Tolcapone tablets (sold as Tasmar and as generic tolcapone) are used as an add-on therapy to levodopa/carbidopa for idiopathic Parkinson's disease — meaning Parkinson's that arises on its own, not from another cause. It is intended for patients who are experiencing symptom fluctuations and are not responding well to, or are not good candidates for, other add-on treatments.

Because of the serious risk of liver failure, tolcapone is reserved for patients who need additional symptom control and must be stopped within 3 weeks if it is not producing a clear benefit.
