Theoretically, taking ashwagandha with hepatotoxic drugs might increase the risk of liver damage.
Ashwagandha has been linked to cases of acute hepatitis, liver failure, hepatic encephalopathy, autoimmune hepatitis, the need for liver transplantation, and death due to liver failure.

Here's the concern with pairing these two. Tolcapone is already known to be tough on the liver. In fact, it carries a strong warning about rare but serious liver injury, which is why people on it usually need regular blood tests to check their liver. Ashwagandha has also been linked to liver problems in some reports, ranging from mild hepatitis all the way to liver failure.

So stacking two things that can each stress the liver may add up. This is a theoretical concern based on how each one behaves, not proven cases of the combo, but with a drug like tolcapone I'd take it seriously. Watch for yellowing of the skin or eyes, dark urine, unusual tiredness, or belly pain, and please check with your doctor or pharmacist before adding ashwagandha.

Ashwagandha is an Ayurvedic herb most often taken to help with stress, anxiety, and sleep, and some small studies suggest it may help, though the evidence is still limited. It is generally well tolerated for short-term use in healthy adults, but it should be avoided in pregnancy and used cautiously by people with thyroid or liver problems. Talk with your pharmacist or doctor before starting it.

About Tolcapone

What Is Tolcapone Used For?

Brand name: Tasmar

Tolcapone tablets (sold as Tasmar and as generic tolcapone) are used as an add-on therapy to levodopa/carbidopa for idiopathic Parkinson's disease — meaning Parkinson's that arises on its own, not from another cause. It is intended for patients who are experiencing symptom fluctuations and are not responding well to, or are not good candidates for, other add-on treatments.

Because of the serious risk of liver failure, tolcapone is reserved for patients who need additional symptom control and must be stopped within 3 weeks if it is not producing a clear benefit.
