Mefenamic Acid falls under ORGANIC ANION TRANSPORTER 3 (OAT3) SUBSTRATES — the group Caffeic Acid interacts with.
In vitro, caffeic acid inhibits OAT3. This drug transport protein is involved in renal tubular uptake of some drugs from the blood and then elimination in the urine. Inhibition of this transporters decreases renal elimination and increases drug levels in the body. Although caffeic acid inhibits OAT3 in vitro, the clinical significance of this in humans is not known.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for mefenamic acid (Ponstel). Your kidneys help clear this medicine out of your body partly through a transporter called OAT3. In lab tests, caffeic acid can block that transporter. In theory, if that blocking happened in real people, mefenamic acid could leave your body a little slower and build up, which might mean stronger effects and a higher chance of NSAID side effects like stomach upset, heartburn, or kidney strain.
Now, the honest part: this is only seen in test-tube studies, so we really don't know if it matters in humans. Everyday amounts from coffee and fruit are very unlikely to be an issue. If you're considering a concentrated caffeic acid supplement, just run it by your pharmacist or doctor first.

Here's the gist with caffeic acid and your mefenamic acid (Ponstel). Your kidneys help clear this NSAID out of your body, and they rely partly on a little transporter called OAT1. In lab dish studies, caffeic acid can slow that transporter down. If that happened in a real person, less of the drug would get flushed out, so levels could creep up and you might see more of the usual NSAID side effects, things like stomach upset, heartburn, or in heavier cases fluid retention or kidney strain.
