Chlorpheniramine, Phenylephrine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Calamus interacts with.

Theoretically, taking calamus with drugs metabolized by CYP3A4 might increase drug levels and potentially increase the risk of adverse effects.
In vitro research shows that calamus extract inhibits CYP3A4 enzyme.

Here's the basic idea with these two combo products (the antihistamine chlorpheniramine plus the decongestant phenylephrine). Your liver uses an enzyme called CYP3A4 to help break them down. In lab dish studies, calamus appears to slow that enzyme. If it does the same thing in people, these medicines could clear out more slowly and build up a bit higher than usual.

What might that feel like? Possibly more of the usual effects: drowsiness, dry mouth, blurry vision, or constipation from the chlorpheniramine, and a faster heartbeat, jitteriness, or higher blood pressure from the phenylephrine. I want to be honest though, this is only a theoretical concern from test-tube data, not proven in humans. Calamus also has real safety worries of its own, so please check with your pharmacist or doctor before combining them.

Theoretically, concurrent use of anticholinergic drugs and calamus might decrease the effectiveness of the anticholinergic drug.

Here's the gist of it. The chlorpheniramine part of this combo works partly by blocking acetylcholine, one of your body's signaling chemicals. Calamus seems to do the opposite in lab studies, where it slows down the enzyme that breaks acetylcholine down, leaving more of it around. In theory, that tug-of-war could blunt how well the antihistamine does its job, so you might notice less relief from a runny nose, sneezing, or itchy eyes.

I want to be honest with you though: this is based only on test-tube research, not real people, so we don't know if it actually plays out that way. The bigger issue is that calamus contains beta-asarone, a compound flagged for safety concerns, so it's one I'd generally steer clear of. Please check with your pharmacist or doctor before combining them.

Calamus is a swamp plant with a long history in Ayurvedic and traditional Chinese medicine, mostly for digestive and nervous-system complaints. However, it contains beta-asarone, a compound linked to cancer and other toxic effects in animal studies, which is why the U.S. FDA banned it from foods. Because of these safety concerns and a lack of solid human evidence, it is generally best avoided.
