Here's the short version for anyone taking Ogsiveo (nirogacestat) for desmoid tumors. Your body relies on a liver enzyme called CYP3A4 to break this medication down. Lab studies suggest calamus can slow that same enzyme, so in theory nirogacestat could clear out more slowly and build up higher in your system. Nirogacestat is not a prodrug, so higher levels would mean stronger effects, and that includes its common side effects like diarrhea, nausea, rash, mouth sores, and fatigue.

I want to be honest about the evidence though: this is based only on test-tube research, not real people, so we can't say how big a change it would actually cause. Calamus also carries its own safety concerns and is best avoided. Please check with your oncology team or pharmacist before combining them.

Calamus is a swamp plant with a long history in Ayurvedic and traditional Chinese medicine, mostly for digestive and nervous-system complaints. However, it contains beta-asarone, a compound linked to cancer and other toxic effects in animal studies, which is why the U.S. FDA banned it from foods. Because of these safety concerns and a lack of solid human evidence, it is generally best avoided.

Ogsiveo (nirogacestat) is approved for adults with desmoid tumors that are actively progressing and require systemic treatment. Desmoid tumors are rare, locally aggressive tumors that can grow in soft tissue.

This indication applies to the oral tablet form of Ogsiveo. It is not approved for children, as safety and effectiveness have not been established in pediatric patients.
