Ranolazine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Calamus interacts with.

Theoretically, taking calamus with drugs metabolized by CYP3A4 might increase drug levels and potentially increase the risk of adverse effects.
In vitro research shows that calamus extract inhibits CYP3A4 enzyme.

Here's the gist for ranolazine (Ranexa): your body relies heavily on a liver enzyme called CYP3A4 to break this medication down. In lab testing, calamus appears to slow that enzyme down. If that happened in a person, ranolazine could build up higher than expected, which may bring on more side effects like dizziness, headache, nausea, constipation, or in some cases changes to your heart's rhythm (a longer QT interval). Ranolazine already carries clear cautions about strong CYP3A4 blockers, so this is worth respecting.

That said, this is only a theoretical, test-tube finding, not something proven in real patients. Even so, given calamus's own safety concerns, I'd skip it and check with your pharmacist or doctor first.

Calamus is a swamp plant with a long history in Ayurvedic and traditional Chinese medicine, mostly for digestive and nervous-system complaints. However, it contains beta-asarone, a compound linked to cancer and other toxic effects in animal studies, which is why the U.S. FDA banned it from foods. Because of these safety concerns and a lack of solid human evidence, it is generally best avoided.

What Is Ranolazine Used For?

Brand names: Ranexa, Aspruzyo

Ranolazine treats chronic angina — the recurring chest pain or pressure that happens when the heart isn't getting enough blood and oxygen. It is used when other angina medicines aren't enough on their own.

Both the extended-release tablet (sold as Ranexa and generic ranolazine) and the oral powder form (sold as Aspruzyo Sprinkle) are approved for this use. Ranolazine can be combined with other heart medicines such as beta-blockers, nitrates, calcium channel blockers, anti-platelet drugs, statins, ACE inhibitors, and angiotensin receptor blockers.
