Macitentan, Tadalafil falls under ANTIHYPERTENSIVE DRUGS — the group Coleus interacts with.
Animal research shows that forskolin, a constituent of coleus, may lower blood pressure.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Opsynvi combines two medicines that already lower blood pressure and relax blood vessels: macitentan (an endothelin receptor blocker) and tadalafil (the same active ingredient as Cialis). Here's the concern with Coleus. Its main compound, forskolin, has been shown in animal and lab studies to relax blood vessels and nudge blood pressure down too. Stack that on top of Opsynvi, and in theory your blood pressure could drop more than intended.
If that happened, you might feel dizzy, lightheaded, flushed, or faint, especially when standing up quickly. To be clear, this is a theoretical interaction, not something proven in people, so I don't want to alarm you. Still, since you're on Opsynvi for a real heart or lung condition, please check with your pharmacist or doctor before adding Coleus.

CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES
2 of 3Macitentan, Tadalafil also falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES.
Research on the effect of coleus on CYP2C9 is conflicting. Some animal research shows that coleus extract can induce CYP2C9, while in vitro research shows that coleus can inhibit CYP2C9. Until more is known, advise patients that taking coleus might increase or decrease levels of drugs metabolized by CYP2C9.

Opsynvi combines macitentan and tadalafil to treat pulmonary arterial hypertension, and both are partly cleared by a liver enzyme called CYP2C9. Here's the honest picture: Coleus (and its active compound forskolin) has only been studied in lab dishes and animals, and the results actually point in opposite directions. Some research suggests it might speed up CYP2C9, while other work suggests it might slow it down.
So we truly don't know which way this would go in a real person. If levels rose, you could see more low blood pressure, dizziness, headache, or flushing. If levels dropped, the medicine might work less well for your lung pressure. On top of that, Coleus itself can lower blood pressure, which may stack with these drugs. This is theoretical, but please run it past your doctor or pharmacist before combining them.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
3 of 3Macitentan, Tadalafil also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
In vitro research shows that coleus can activate the nuclear receptor, pregnane X receptor (PXR), which results in increased expression of CYP3A4. Although the clinical significance of this is not known, use caution when considering concomitant use of coleus and other drugs affected by these enzymes.

Opsynvi combines two medicines, macitentan and tadalafil, that help lower the high blood pressure in your lungs (pulmonary arterial hypertension). Both are broken down by a liver enzyme called CYP3A4. In lab studies only, Coleus (forskolin) seems to nudge the body into making more of that enzyme. If that happened in real life, it could clear these drugs a little faster and make them less effective, which might mean more shortness of breath, fatigue, or trouble with exertion over time.
I want to be honest though: this is based on test-tube data, not people, so we really don't know if it matters. Since PAH is serious and Coleus can also affect blood pressure on its own, please check with your pharmacist or doctor before combining them.
