Niraparib, Abiraterone falls under HEPATOTOXIC DRUGS — the group Comfrey interacts with.

Theoretically, comfrey might have additive adverse effects on the liver when used with hepatotoxic drugs.
Due to its pyrrolizidine alkaloid constituents, comfrey can cause hepatotoxic effects, including ascites, cirrhosis, hepatic fibrosis, hepatomegaly, and sinusoidal obstruction syndrome.

Here's the simple version. Akeega (the combination of niraparib and abiraterone) is a cancer treatment that already asks a lot of your liver. Doctors routinely check your liver enzymes while you're on it, because abiraterone in particular can raise those numbers and, in some people, cause real liver strain. Comfrey taken by mouth is a known liver toxin thanks to compounds called pyrrolizidine alkaloids, which have been linked to serious liver damage.

So the worry here is additive stress on the same organ. This concern is theoretical, based on how each one behaves rather than proven cases together, but it points the same direction. I'd steer clear of oral comfrey entirely while on Akeega, and even skin products deserve a quick check. Please loop in your oncology team or pharmacist before using any comfrey.
