Nitrofurantoin falls under HEPATOTOXIC DRUGS — the group Comfrey interacts with.

Theoretically, comfrey might have additive adverse effects on the liver when used with hepatotoxic drugs.
Due to its pyrrolizidine alkaloid constituents, comfrey can cause hepatotoxic effects, including ascites, cirrhosis, hepatic fibrosis, hepatomegaly, and sinusoidal obstruction syndrome.

Here's the concern with pairing these two. Comfrey contains natural compounds called pyrrolizidine alkaloids that can be hard on the liver, especially when it's swallowed rather than used on the skin. Nitrofurantoin, your UTI antibiotic, also carries a known (though uncommon) risk of liver irritation. Put them together and the worry is simply that two liver stressors could add up.

To be clear, this is a theoretical caution based on how each one behaves, not strong proof from studies. Nitrofurantoin isn't a prodrug, so comfrey wouldn't make the antibiotic stronger or weaker. The real issue is liver safety. Comfrey should never be taken by mouth anyway. If you're on nitrofurantoin and want to use a topical comfrey product, just run it by your pharmacist or doctor first.

Comfrey is a traditional herb used mainly on the skin for bruises, sprains, and joint pain, and some topical products show modest benefit for these uses. However, comfrey contains compounds that can seriously damage the liver, so it should never be taken by mouth and should only be used on unbroken skin for short periods. Pregnant or breastfeeding people should avoid it entirely.

About Nitrofurantoin

What Is Nitrofurantoin Used For?

Brand names: Macrodantin, Furadantin, Furatoin, Macrobid

Nitrofurantoin is used to treat urinary tract infections (UTIs) caused by certain susceptible bacteria. The capsule forms (including Macrodantin) are indicated for UTIs due to susceptible strains of Escherichia coli, enterococci, Staphylococcus aureus, and certain strains of Klebsiella and Enterobacter species. Nitrofurantoin oral suspension (Furadantin) is indicated for adults and children 1 month of age and older for the same types of UTIs.

Nitrofurantoin is not used for kidney infections (pyelonephritis) or infections of the tissue around the kidney (perinephric abscesses). Because it works primarily in the urine and does not spread widely through body tissues, it may not fully clear the infection in some patients, and the UTI may return after treatment.
