Riluzole falls under HEPATOTOXIC DRUGS — the group Comfrey interacts with.

Theoretically, comfrey might have additive adverse effects on the liver when used with hepatotoxic drugs.
Due to its pyrrolizidine alkaloid constituents, comfrey can cause hepatotoxic effects, including ascites, cirrhosis, hepatic fibrosis, hepatomegaly, and sinusoidal obstruction syndrome.

Here's the thing about riluzole: it's already known to be a little tough on the liver. In fact, your doctor likely checks your liver enzymes with blood tests, especially during your first months on it. Comfrey is the concern here because, when taken by mouth, its natural pyrrolizidine alkaloids can also harm the liver. Stacking two liver stressors together could, in theory, add up and raise your risk of liver trouble.

To be fair, this is a theoretical concern based on how each one behaves, not solid proof from studies. Still, I'd play it safe. Never swallow comfrey, and even short-term use on the skin is worth running past your doctor or pharmacist first while you're on riluzole.

Comfrey is a traditional herb used mainly on the skin for bruises, sprains, and joint pain, and some topical products show modest benefit for these uses. However, comfrey contains compounds that can seriously damage the liver, so it should never be taken by mouth and should only be used on unbroken skin for short periods. Pregnant or breastfeeding people should avoid it entirely.

What Is Riluzole Used For?

Riluzole is approved to treat amyotrophic lateral sclerosis (ALS) — the progressive neurological disease also known as Lou Gehrig's disease. This indication is shared across all available riluzole products, including tablets (Rilutek, riluzole), the dissolving oral film (Exservan), and the oral suspension (Tiglutik).

Tiglutik oral suspension is notable in that it can also be given through a PEG-tube (percutaneous endoscopic gastrostomy tube) for patients who have difficulty swallowing.
