Modafinil falls under CYTOCHROME P450 3A4 (CYP3A4) INDUCERS — the group Comfrey interacts with.

Theoretically, CYP3A4 inducers might increase the risk of adverse effects from the pyrrolizidine alkaloid constituents in comfrey.
CYP3A4 enzymes convert pyrrolizidine alkaloids, constituents of comfrey, to toxic metabolites. Some case reports show that enzyme inducers, such as phenobarbital, seem to enhance the toxicity of comfrey.

Here's the key thing to understand about this pairing: it's really comfrey's safety that's the concern, not how well your Modafinil works. Modafinil gently nudges up the activity of a liver enzyme called CYP3A4. That same enzyme is what turns the natural compounds in comfrey (called pyrrolizidine alkaloids) into byproducts that can be toxic to the liver. So in theory, taking Modafinil could make comfrey a bit harder on your liver than it would be on its own.

I want to be honest that this is a theoretical worry based on how these substances behave, plus a few older case reports with other enzyme boosters. It's not a strong, proven interaction. Still, comfrey is risky for the liver to begin with and should never be swallowed. If you're using it on your skin, that's far less of an issue, but please run it by your pharmacist or doctor first.

Comfrey is a traditional herb used mainly on the skin for bruises, sprains, and joint pain, and some topical products show modest benefit for these uses. However, comfrey contains compounds that can seriously damage the liver, so it should never be taken by mouth and should only be used on unbroken skin for short periods. Pregnant or breastfeeding people should avoid it entirely.

Modafinil helps adults stay awake when they have excessive sleepiness linked to narcolepsy, obstructive sleep apnea (OSA), or shift work disorder (SWD). Both Provigil and generic modafinil tablets are labeled for these three uses.

In OSA, it treats the sleepiness only. It does not treat the airway blockage itself. If CPAP (a breathing machine) is your treatment of choice, a full effort to use it should come first and continue during treatment.
