Due to its pyrrolizidine alkaloid constituents, comfrey can cause hepatotoxic effects, including ascites, cirrhosis, hepatic fibrosis, hepatomegaly, and sinusoidal obstruction syndrome.

Here's the simple version: both comfrey and azathioprine can be hard on your liver, so using them together may add up. Azathioprine is already known to sometimes raise liver enzymes or cause liver inflammation, which is why your doctor checks bloodwork while you take it. Comfrey contains pyrrolizidine alkaloids that can directly injure the liver, which is why it should never be swallowed.

This concern is theoretical, based on how each one behaves rather than reported cases, so I don't want to alarm you. Still, the safest move is to avoid comfrey taken by mouth completely. Even short-term use on unbroken skin is best run past your pharmacist or doctor first, since you're already on a liver-sensitive medication.

Comfrey is a traditional herb used mainly on the skin for bruises, sprains, and joint pain, and some topical products show modest benefit for these uses. However, comfrey contains compounds that can seriously damage the liver, so it should never be taken by mouth and should only be used on unbroken skin for short periods. Pregnant or breastfeeding people should avoid it entirely.

About Azathioprine

What Is Azathioprine Used For?

Brand names: Imuran, Azamune

Azathioprine is used alongside other medications to help prevent the body from rejecting a kidney transplant (renal homotransplantation). It is also used to treat active rheumatoid arthritis — reducing signs and symptoms in adults who have not responded well enough to other treatments.

Other pain relievers such as aspirin, NSAIDs, or low-dose steroids may be continued alongside azathioprine for rheumatoid arthritis. Combining azathioprine with disease-modifying antirheumatic drugs (DMARDs) has not been well studied and is not recommended.
