Why this applies to Bedaquiline

Bedaquiline falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Cranberry interacts with.

Theoretically, cranberry might increase the levels and adverse effects of CYP3A4 substrates.
A case of upper back pain, rhabdomyolysis, and abnormal liver function has been reported for a patient taking atorvastatin, a CYP3A4 substrate, in combination with cranberry juice 16 ounces daily for 2 weeks. Creatinine kinase and liver enzymes normalized within 2 weeks of stopping cranberry juice. Also, animal research suggests that cranberry juice, administered intraduodenally 30 minutes prior to nifedipine, a CYP3A4 substrate, inhibits nifedipine metabolism and increases the area under the concentration-time curve by 1.6-fold compared to control.

Here's the idea behind this one: your body clears bedaquiline mainly through a liver enzyme called CYP3A4. Cranberry, in concentrated amounts, might slow that enzyme down a little. If that happens, bedaquiline could build up a bit more than expected, which in theory could nudge up the risk of its known side effects, like heart rhythm changes (a longer QT interval) or liver irritation.

I want to be honest about the evidence, though: this is mostly theoretical, based on a single case with a different drug and some animal data. A glass of cranberry juice now and then is unlikely to be a problem. Daily, concentrated cranberry supplements are where I'd be more cautious. Since bedaquiline is a serious TB medication, please run any cranberry product past your TB team or pharmacist first.

What Is Bedaquiline Used For?

Brand name: Sirturo

Sirturo (bedaquiline) is used as part of a combination treatment for pulmonary TB — TB that affects the lungs — caused by bacteria resistant to at least both rifampin and isoniazid. It is approved for adults and for children 2 years and older who weigh at least 8 kg (about 17.6 lbs).

Sirturo is not used for latent TB (TB infection with no active symptoms), TB outside the lungs, drug-sensitive TB, or infections caused by non-tuberculosis mycobacteria. It must always be used alongside other TB medicines — never alone.
