Acetaminophen, Dextromethorphan, Pseudoephedrine falls under CYTOCHROME P450 2E1 (CYP2E1) SUBSTRATES — the group Delta-9-tetrahydrocannabinol (thc) interacts with.

Theoretically, THC might decrease the levels and clinical effects of CYP2E1 substrates.
In vitro research shows that cannabis containing THC can induce the activity of CYP2E1, which might increase the metabolism of CYP2E1 substrates. It is unclear if this effect is due to THC, other constituents, or the combination.

This combo product mixes three ingredients, but the piece that matters here is the acetaminophen (Tylenol), which is partly handled by a liver enzyme called CYP2E1. The theory on file is that THC might speed that enzyme up a little, which could shift how acetaminophen is processed. Honestly, this is based only on lab and animal data, so it's pretty far from proven in real people.

One thing worth knowing: when acetaminophen runs through CYP2E1 faster, it can make slightly more of a byproduct that's hard on the liver, especially with high doses or regular alcohol use. I wouldn't lose sleep over an occasional cold tablet, but stick to the labeled dose. If you use cannabis regularly or take acetaminophen often, check in with your pharmacist or doctor.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Acetaminophen, Dextromethorphan, Pseudoephedrine also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, THC may increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that cannabis containing THC might modestly inhibit the activity of CYP3A4 enzymes, which might decrease the metabolism of CYP3A4 substrates. It is unclear if this effect is due to THC, other constituents, or the combination. In vitro research also shows that cannabis extracts containing THC modestly inhibit the CYP3A4 metabolism of testosterone; extracts providing the specific cannabinoids cannabidiol (CBD) or cannabigerol (CBG) alone had stronger inhibitory effects than extracts containing both THC and CBD. Conversely, a crossover clinical study in healthy adults shows that oral THC does not inhibit CYP3A4.

Let me put your mind at ease here. The thinking is that THC might slightly slow down a liver enzyme called CYP3A4, which helps clear the dextromethorphan (the cough suppressant) part of this combo cold product. In theory, that could let a little more dextromethorphan hang around, possibly adding to drowsiness or feeling foggy. THC can already make you feel sleepy or off, so you might just feel a bit more of that.

That said, this is a very weak concern. It comes from lab dish studies, and a human study found oral THC did not actually block CYP3A4. The acetaminophen and pseudoephedrine pieces aren't really affected. Still, it's smart to run it by your pharmacist before combining them.
