Why this applies to Bosentan

Bosentan falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Delta-9-tetrahydrocannabinol (thc) interacts with.

Theoretically, THC might increase the levels and adverse effects of CYP2C9 substrates.
In vitro research shows that THC moderately inhibits the CYP2C9-mediated 7-hydroxylation of S-warfarin in a concentration-dependent manner. In vitro research also shows that cannabis extracts containing THC modestly inhibit the CYP2C9 metabolism of tolbutamide; extracts providing the specific cannabinoids cannabidiol (CBD) or cannabigerol (CBG) alone had stronger inhibitory effects than extracts containing both THC and CBD. Theoretically, THC may inhibit the metabolism of other CYP2C9 substrates. Conversely, a crossover clinical study in healthy adults shows that oral THC does not inhibit CYP2C9.

Here's the gist for bosentan (Tracleer): your liver partly clears it using an enzyme called CYP2C9. In lab dish studies, THC has shown a mild ability to slow that enzyme down. If that happened in real life, bosentan could build up a little, which in theory might mean more of its usual side effects, things like headache, flushing, swelling in the legs, or low blood pressure that leaves you feeling dizzy.

That said, I want to keep this in perspective. This is graded as a minor and unlikely interaction, based only on test-tube and animal data. In fact, one human study found oral THC did not block CYP2C9 at all. So I wouldn't lose sleep over it, but do mention any THC use to your pharmacist or doctor, since bosentan needs careful monitoring anyway.

Theoretically, THC may increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that cannabis containing THC might modestly inhibit the activity of CYP3A4 enzymes, which might decrease the metabolism of CYP3A4 substrates. It is unclear if this effect is due to THC, other constituents, or the combination. In vitro research also shows that cannabis extracts containing THC modestly inhibit the CYP3A4 metabolism of testosterone; extracts providing the specific cannabinoids cannabidiol (CBD) or cannabigerol (CBG) alone had stronger inhibitory effects than extracts containing both THC and CBD. Conversely, a crossover clinical study in healthy adults shows that oral THC does not inhibit CYP3A4.

Here's the gist with bosentan (Tracleer) and THC. Your liver uses an enzyme called CYP3A4 to help clear bosentan. Lab (test tube) studies hint that THC, or other parts of cannabis, might slightly slow this enzyme down. In theory, that could let a bit more bosentan stay in your system, which might mean more of its usual effects like headache, flushing, swelling in your legs, or feeling tired.

That said, I want to keep this in perspective. This is rated as a minor and unlikely interaction, and it's based only on lab data, not real people. In fact, one human study found oral THC did not block CYP3A4 at all. So this is mostly theoretical. Still, since bosentan is a serious medication, it's worth mentioning your cannabis use to your pharmacist or doctor so they can keep an eye on things.

What Is Bosentan Used For?

Brand name: Tracleer

Bosentan is used to treat pulmonary arterial hypertension (PAH) — high blood pressure in the arteries of the lungs. In adults, it helps improve the ability to exercise and slows how quickly the disease gets worse.

In children aged 3 years and older with idiopathic (no known cause) or congenital (present from birth) PAH, bosentan is used to lower resistance in the lung blood vessels, which is expected to improve exercise ability. Tracleer is one brand approved for both adults and pediatric patients with PAH.
